Helping people stay well while remaining independent is a key priority for health and social care organisations in Norfolk.
To achieve this, work is taking place across the county to ensure people have access to the care and support they need within or closer to their own homes.
One important element of this work is the Supported Care Service in North Norfolk and rural Broadland, which is seeking to reshape care around local needs.
The model is being developed in close partnership by NHS South Norfolk Clinical Commissioning Group , Norfolk Community Health and Care NHS Trust, and Norfolk County Council.
